2025-26 K-8 Application Submission

Thank you for applying to St. Louis Catholic School. All information, forms and payments noted with an asterisk must be submitted for each applicant for an Admissions Packet to be initially placed in the queue for consideration for acceptance. Additional signatures and documentation, where applicable, must also be submitted. Families are responsible for submitting the Student Evaluation form and Release of Student Records form to their child's current school as well as scheduling a tour and shadow day (Middle School applicants) to complete their Admissions Packet.

Applications are only considered in the order they are received by priority category if the complete Admissions Packet has been received by February 1, 2025. Applications received after this date will only be considered should space be available. Admissions Packets missing documentation after this date may be removed from the queue until the requirements are fulfilled.

Please attach the following admissions forms and documentation. Those noted with an asterisk are required.

Acceptable file formats include PDF, .doc, .jpg and .png.

Name each of your files with your child's first initial, last name and name of the document. Example: JSmith_Application, JSmith_Faith, JSmith_BirthCert, JSmith_Baptism, etc.

4-page application with all required signatures on pages 3 and 4. Applications missing signatures will not be considered complete. Signatures of both parents listed on the birth certificate are required on the Admissions Application. If a parent is deceased, please note that on page 2 of the application. Parents who are separated or divorced must submit proof of custody or transfer of legal Guardianship (upload in the appropriate field below).

5th page of the application. Applicants seeking Saint Louis Parishioner or Catholic priority for application status and tuition rates must have this form signed by a representative of their parish. Forms received unsigned will be considered inactive or non-Catholic applicants.

Baptismal Certificate and Relevant Sacramental Documentation
Catholic applicants must submit a Baptismal certificate. Application packets that do not have a Catholic Baptismal certificate included will be considered non-Catholic for applicant priority status purposes. Please note, Catholic tuition rates are determined based upon the child's Baptismal status. Documentation of other sacraments received (Reconciliation, First Communion and Confirmation) may also be uploaded.

If you are requesting an adjustment or accommodation to allow participation to any program, please describe your request on a separate sheet of paper. Application Fee

All admisisons packets must be accompanied by a non-refundable application fee of $60.00. Credit or debit cards are acceptable forms of payment.
